Does Creatine Cause Hair Loss? What the 2025 Research Actually Says

No, creatine does not cause hair loss in healthy adults. A 2025 randomized controlled trial published in the Journal of the International Society of Sports Nutrition directly measured hair follicle health during 12 weeks of creatine use and found no significant differences in DHT, testosterone, or hair growth parameters between the creatine group and placebo. This was the first study to directly assess hair follicles, and it settled the debate: creatine at standard doses does not cause hair loss.

The confusion goes back to one small 2009 study in rugby players. That study, its limitations, and the 15 years of internet panic that followed are the actual story.

Here’s what the research shows, why the myth persisted, and who (if anyone) still has a reason to think about this.

If you have male pattern baldness in your family: Genetics drives your hair loss, not creatine. If you’re already thinning, creatine won’t accelerate it in any measurable way based on current evidence.

If you’re worried anyway: Skip creatine and lose 5-15% of your training output, or take creatine and get the strength gains. Your call. The science says the trade-off isn’t real.


Where the myth actually came from

Every “creatine causes hair loss” article on the internet traces back to one paper: van der Merwe et al. 2009, published in the Clinical Journal of Sport Medicine.

Here’s what actually happened in that study.

Twenty college-aged male rugby players were randomized to either creatine or placebo. The creatine group loaded with 25g per day for 7 days, then took 5g daily for 14 more days. Total study length: 3 weeks.

The finding that started everything: after 7 days of loading, the creatine group’s dihydrotestosterone (DHT) levels rose by 56% from baseline. After the 14-day maintenance phase, DHT was still 40% above baseline in the creatine group compared to placebo.

That sounds alarming. Here’s what got lost in the internet game of telephone that followed:

The DHT levels never left the normal range. Baseline was 0.98 nmol/L. After creatine, DHT rose to 1.26 nmol/L. Normal range in adult men is roughly 0.86-2.9 nmol/L. Both values were completely healthy. The percentage change looked scary. The absolute values did not.

Testosterone didn’t change. Only DHT rose. This is worth noting because DHT is derived from testosterone via the 5-alpha-reductase enzyme. A rise in DHT without a rise in testosterone is a slightly odd finding.

Hair loss was never measured. The study never actually looked at hair follicles, hair density, or hair growth. It measured a hormone associated with male pattern baldness in genetically susceptible people. That’s an inference, not a finding.

Sample size was 20 people, 3 weeks long. For a claim to become “creatine causes hair loss,” you’d want much more than that.

The study has never been replicated. In the 15+ years since, no other study has reproduced the DHT-rise finding.

That’s the origin. One small study. Never replicated. Never actually measured hair. Somehow became “creatine causes hair loss” on every fitness forum on the internet.


The study that settled it: 2025 RCT

The evidence got a lot cleaner in April 2025 when Antonio et al. published a 12-week randomized controlled trial in the Journal of the International Society of Sports Nutrition specifically designed to answer this question.

What made this study different:

They actually measured hair follicles. Not just hormones. Board-certified dermatologists at the Iran Skin Research Center used the Trichogram test and the FotoFinder system to directly assess hair growth parameters, hair density, follicular unit count, and hair thickness.

12 weeks, not 3. Long enough to see meaningful hair changes if they were happening.

Resistance-trained men, ages 18-40. Real gym-goers, standard 5g daily dose.

Blinded, placebo-controlled. Neither participants nor assessors knew who got which.

The results:

  • No significant difference in DHT levels between creatine and placebo groups
  • No significant difference in the DHT-to-testosterone ratio
  • No changes in any hair growth parameter (density, follicular unit count, thickness)
  • No changes in testosterone or free testosterone

The authors’ conclusion: this study “provides strong evidence against the claim that creatine contributes to hair loss.”

That’s about as clear as sports nutrition research gets.


What about the other 13+ studies?

The 2025 RCT wasn’t the first to look at creatine and hormones. It was the first to look at hair follicles directly. But there’s a decade of hormone research that had already made the case:

Examine.com’s analysis of the full literature: 12 of 13 studies that measured creatine’s effect on testosterone found no significant change. Five of those studies also measured free testosterone, and none found a change. Only the 2009 rugby study reported a DHT increase, and even that stayed within normal range.

A 2021 comprehensive review in the Journal of the International Society of Sports Nutrition concluded that the evidence for creatine causing hair loss is “not compelling” and that the “purported link between creatine and hair loss is not supported by the scientific literature.”

The Cleveland Clinic’s position (updated 2024): “no conclusive evidence suggests that creatine increases testosterone or causes hair loss.”

GoodRx’s position (April 2026): “creatine doesn’t cause hair loss in either males or females.”

Every major clinical source has arrived at the same conclusion. The 2025 RCT just made it official with direct measurement.


Why the myth won’t die

Even with the research clear, this myth persists in gym conversations, Reddit threads, and fitness YouTube. There are a few reasons for that.

Anecdotal reports feel real. Some men start creatine, notice their hair thinning, and connect the two. What’s actually happening in most cases: they’re 25-35 years old, right in the demographic where male pattern baldness starts. The hair loss was going to happen. Creatine was a coincidence.

Fear of DHT is exaggerated. DHT gets discussed in bodybuilding circles as if it’s a pure villain. In reality, DHT is essential for a lot of normal biological function (libido, mood, cognitive performance). Blocking DHT with 5-alpha-reductase inhibitors (like finasteride) is a legitimate treatment for hair loss, but it comes with real side effects. The idea that any DHT rise is dangerous conflates two different things.

The 2009 study makes for a spicier headline than the 2025 study. “Creatine raises DHT by 56%” is a click. “12-week trial shows no effect on hair” is not. Media has kept the 2009 study alive.

Nobody has an incentive to fix the record. Supplement brands can’t loudly say “creatine is safe for your hair” without triggering “you would say that.” Independent fitness creators worry about liability if they overpromise. Only the peer-reviewed journals push back, and most people don’t read those.


The one situation where you should still think about it

There’s a narrow case where hair-loss concern with creatine could be legitimate, and it’s worth being honest about:

Men with active androgenetic alopecia (male pattern baldness) already in progress, with a strong family history.

Here’s the reasoning:

For genetically susceptible individuals, hair follicles in the crown and hairline are sensitive to DHT. Even normal-range DHT can bind to follicle receptors and drive the miniaturization process that causes thinning. If you’re already in that progression, anything that adds even minor stress to the system could theoretically nudge things faster.

The 2025 RCT didn’t specifically enroll men with active androgenetic alopecia. Its participants were healthy resistance-trained men, some with family history, some without. That’s a limitation the study authors acknowledged.

So if you’re in your late 20s or 30s, actively watching your hairline recede, and your dad and grandfather both went bald, the question isn’t fully closed for your specific situation.

Practical advice for that specific case:

  1. See a dermatologist before making decisions. If you’re seriously worried about hair, you probably need finasteride or minoxidil, not to skip creatine.
  2. If you decide to take creatine, note that you’re likely gaining more from strength training with saturation than you’d lose from any theoretical hair effect. The training benefit is measurable. The hair effect, if any, is speculative.
  3. If you want to be maximally cautious, use a lower daily dose (3g instead of 5g). It’s not clinically ideal for saturation speed, but it may address anxiety about the DHT question while still giving most of the benefit over time.

For 95% of people asking this question, that entire section doesn’t apply. Take the 5g. Get on with training.


What actually causes hair loss (for context)

Since we’re here: hair loss in men and women has real drivers. Creatine isn’t on the list. What is:

Genetics. Androgenetic alopecia (male pattern baldness and female pattern hair loss) is the largest single cause of hair loss, driven by inherited sensitivity of hair follicles to DHT. Not by DHT levels themselves.

Age. Hair follicle density decreases naturally over time.

Nutritional deficiencies. Iron deficiency, protein deficiency, and certain vitamin deficiencies (D, B12, biotin) can trigger hair shedding.

Chronic stress. Elevated cortisol drives telogen effluvium, the shedding pattern most commonly seen in high-stress life periods.

Thyroid disorders. Both hyper and hypothyroidism can cause hair thinning.

Medications. Certain antidepressants, blood pressure meds, and acne treatments have hair loss side effects.

Rapid weight loss or extreme cutting. Aggressive calorie restriction can trigger telogen effluvium.

Autoimmune conditions. Alopecia areata specifically.

If you’re a lifter concerned about hair, the productive things to focus on are: adequate protein (which you’re doing if you take whey), iron and vitamin D status (get bloodwork if you’re worried), stress management, and sleep. Creatine isn’t the lever to pull.

Does creatine cause acne?

Not directly. Some lifters report breakouts starting creatine, but the mechanism is usually indirect: harder training leads to more sweat, more oil production, and more mechanical rubbing (from headbands, straps, chalk). Creatine itself doesn’t drive sebum production in any documented way. If you’re breaking out, wash more thoroughly post-workout and consider your training clothes hygiene.

Does creatine cause bloating?

Sometimes, especially during loading. The mechanism: creatine pulls water into muscle cells, and during the 20g/day loading phase some of that water can cause temporary GI discomfort or a bloated feeling. Skip loading and start at 5g daily to avoid this. Most people who skip loading never notice bloating.

Does creatine cause water retention?

Yes, but not in a bad way. Creatine draws water into your muscle cells (intracellular hydration). This adds about 1-3 pounds of scale weight in the first weeks. That water is inside your muscles, not under your skin. It doesn’t look like bloat. It looks like fuller muscles. If anything, it improves your physique.

Does creatine cause kidney damage?

No, in healthy adults. This is the other zombie myth of creatine research. The ISSN Position Stand on Creatine reviewed decades of research and concluded that creatine does not cause kidney damage in healthy people at recommended doses (5g/day) over long-term use. People with pre-existing kidney disease should consult a doctor before supplementing.

The confusion comes from the fact that creatine metabolism produces creatinine, which shows up on kidney function tests. Higher creatinine on a blood test can look like impaired kidney function to a doctor who doesn’t know you supplement. Tell your doctor you take creatine before any blood work.

Does creatine make you look bloated in the face?

No. The face has different tissue than skeletal muscle. Creatine doesn’t cause facial water retention. If you look puffy, it’s probably alcohol, salt, or sleep quality, not creatine.


Should men worried about hair loss take creatine?

The honest answer: yes, unless you specifically fall into the “active androgenetic alopecia with strong family history” category discussed above.

For everyone else:

  • The research shows creatine doesn’t cause hair loss
  • The training benefits (strength, muscle mass, work capacity) are measurable and real
  • The theoretical hair risk is speculative
  • Skipping creatine to protect hair you were never going to lose is a bad trade

If you’re actively balding and terrified, see a dermatologist. Get on finasteride or minoxidil if that’s appropriate for you. Take creatine while you address the actual driver. Don’t blame a supplement for genetics.


What about women and creatine?

Women often ask if creatine’s DHT concerns apply to them. Short answer: female pattern hair loss has different mechanisms than male pattern baldness. The DHT concern is less central for women.

The 2025 RCT only studied men, so we don’t have equivalent direct data on women. However, given that the hormonal effect wasn’t observed in men, extrapolating to women (who have lower baseline testosterone and DHT anyway) suggests even less risk.

Women who want to take creatine for the well-documented strength, muscle mass, and cognitive benefits shouldn’t hesitate based on hair loss concerns.

Can I take creatine while on finasteride? Yes, there’s no known interaction. Finasteride blocks the enzyme that converts testosterone to DHT. Since creatine doesn’t meaningfully change DHT anyway, there’s no theoretical conflict. Consult your doctor about any drug combination questions specific to your health.

Does whey protein cause hair loss? No. Whey protein doesn’t cause hair loss. The hormonal effects sometimes cited for whey (IGF-1 elevation, mild insulin response) are within normal dietary variation and haven’t been linked to hair loss in controlled studies.

How long after starting creatine do I need to worry about hair? You don’t need to worry. But if you want to monitor, take photos of your hairline monthly for the first 6 months. Compare directly. Any change is far more likely to be normal aging or unrelated factors than creatine.
